As part of PACE (Program of All-Inclusive Care for the Elderly), you'll deliver truly comprehensive care — medical, social, and emotional — to older adults who need it most. You’ll work closely with a dedicated interdisciplinary team including nurses, on-site social work, physical therapy, occupational therapy, registered dietician, day center coordinator, home care, transportation, and more.

What You’ll Do

  • Serve as primary care physician for PACE participants (typically 55+)
  • Provide preventative, acute, and chronic care with a focus on aging with dignity
  • Collaborate with the interdisciplinary team to coordinate holistic care plans
  • Build trusted relationships with participants and their families
Qualifications

Who are we looking for?

  • MD or DO with a valid California license (or eligibility)
  • Board-certified/eligible in Internal Medicine or Family Medicine
  • Fellowship in Geriatrics, Hospice and Palliative Medicine, or relevant experience in geriatric medicine encouraged
  • Passion for working with underserved older adult populations
